Here are the simple steps to apply for an SSS ID Card via SSS's Online Appointment System:
1. Log in to your SSS account online via this link here: SSS Member Login.
2. After successfully logging in, you will be directed to this page below and simply click on the "Appointment System" link.
3. Choose your preferred date of appointment.
4. Fill up the necessary details for your appointment.
5. After clicking the Submit button, you will then receive a confirmation email about your scheduled appointment. Kindly bring a print out of the confirmation email upon going to the assigned SSS branch.
6. On your scheduled appointment, go there on the scheduled time and line up at the Public Assistance Desk or the Forms/Priority Nos. Desk and inform the staff that you have a scheduled appointment via their online system.
7. Prepare 2 valid IDs with photocopy, completely signed E-6 form(you can download it here) as well as the printout of the confirmation email. You will then be given a priority slip shown below:
8. Look for the security personnel assigned in the "General Transactions or General Section" and present your slip. He/she will direct you to your assigned line to where you'll submit your E-6 form and photocopy of your IDs.
9. After successful validation of your requirements, you'll be advised to proceed to the ID Capturing area.
10. Submit your validated documents and have your photo and signature taken.
11. You're done. You'll be advised to wait for atleast 3 months for your SSS ID to be delivered on your residential address. After the 3 months has lapsed and your still unable to receive your ID, follow it up at your nearest post office.

ReplyDeletebased on my experience yes, because i didn't need to fall in line and be early. all i did was bring the requirements and show up before my scheduled appointment time which was at 1pm

ReplyDeletei search the net & found written in your company's official internet site as of jan.15 2014. stated there for only a week as in 7 working days to 10 working days!!! from the day of application + another week for maiing!!! all applicants will receive the sss i.d unified umid card!!! what happen!? and beesides that both my sister & neice got theirs w/in a week!!! IT'S A HEX!!! i need my card for processing important papers but i cant make a move all because of the i.d!!! P-Noy no changes still in the gov.!!!
